CommCare's clients include member agencies, health maintenance organizations, governmental entities and employers.
Background Statement Community Network for Behavioral Healthcare, Inc. (CommCARE) was formed in March of 1995 by nine NW Region area community mental health centers and substance abuse agencies in response to the Missouri Medicaid, Managed Care Plus (MC+) program implemented September 1, 1995. By forming a regional provider-sponsored integrated delivery system, CommCARE was positioned to negotiate and secure capitated agreements with pre-paid health plans. CommCARE exited the Missouri Managed Care program in 2007 but has continued to provide utilization management services for health plans in other areas. CommCARE’s Board of Directors is comprised of the Chief Executives from the Community Mental Health Agencies (CMHC’s) and substance abuse agencies (SAA’s) as designated by the Missouri Department of Mental Health.
Impact Statement CommCare is committed to improving lives through quality Behavioral Health Services. CommCARE improves access to services by developing relationships with members and employers. CommCARE nurtures and cultivates relationships with members and employer, and offers flexibility and responsiveness to needs.
Needs Statement
CommCARE seeks to expand its capability to better analyze health outcome trends to support community members and healthcare providers. CommCARE wishes to improve access to care by precisely connecting people in a crisis to valuable community resources. CommCARE can also connect those wishing to help a person in a crisis with community resources. Finally, CommCARE desires to augment the programs and services of 28 community behavioral health and substance use providers to meet the most urgent and prevalent behavioral health needs.
CommCARE's most pressing needs:
  • Secure ongoing funding for FSFH call center operations
  • Evolve and Respond to community needs for youth mental health and suicide prevention by adding chat/texting features to FSFH
  • Continue to promote FSFH to the community through an enhanced marketing and public relations campaign
  • Enhance operations through increased volunteer and administrative support

Description CommCARE provides 24/7 ACI crisis services to residents in 22 counties and seven community mental health organizations in the northwest region of Missouri for the Missouri Department of Mental Health. CommCARE provides 24/7 crisis services to callers for 816 and 660 area for the National Suicide Prevention Lifeline.

Examples of Program Success During FY19 First Step for HELP served over 18,000 individuals. First Step for HELP is a National Suicide Prevention Lifle, the third in Missouri. This program is CARF accredited.
Description An Opioid overdose response program designed to target individuals who present to local Emergency Departments after an opioid overdose. It is a collaboration with the Missouri Department of Mental Health. Once identified, Certified Peer Specialist (CPS) Recovery Coaches will be dispatched to six local emergency room departments in order to engage with the client and offer information and referral to follow up services as necessary. In addition to connection to six local MH and SUD recovery agencies the program works on a medication first model to help connect those suffering from opioid use disorder with Medication Assisted Recovery. Each patient referred to our program will also receive Overdose Education and Narcan Distribution.

Examples of Program Success The program has been in operation for three months receiving 13 referrals for services.
Description CommCARE provides operational oversight and utilization management in a collaboration between the Kansas City Metropolitan Stakeholders and the Missouri Department of Mental Health. The program provides intensive services to high-risk children to keep children in the home and reduce the rotation in and out of inpatient mental health services.

Examples of Program Success During FY19 the program served 14 children this year. A minimum of 12 hospitalizations were potentially prevented by having staff in the home.
Description CommCARE providers are available to individual ages 6 years and older as of April 2018. Clients served are enrolled with CommCARE’s partnering community mental health centers.

Examples of Program Success During FY19 CommCARE expanded the program by adding an APRN.
Description CommCARE provides single-point-of-contact assessment and referral services which requires development and maintenance of a nationwide provider network. Referrals are provided for mental health, legal consultation, financial counseling, coaching, Critical Incident Response, and resources for child and elder care.

Executive Director Mr. Terry Trafton
Term Start June 2017
Mr. Trafton has over 25 years of experience in healthcare settings including behavioral health, community and hospital healthcare. Trafton worked for Two Rivers Behavioral Health System of UHC Inc. in 2016 as Chief Executive Officer/Managing Director. For 19 years, he works worked at Truman Medical Center’s Behavioral Health Division including eight years as an Administrator. Trafton holds a National Certification and License as a Professional Counselor in the State of Missouri with a Master’s degree from Avila University. He is also a member of the American College of Healthcare Executives.
Trafton serves as the current Chair on the Missouri Mental Health Foundation Board of Directors, a Board Member of the Grater Lee’s Summit Healthcare Foundation and current chair on the Health Forward Foundation’s Advisory Council. Trafton is also a member of the Campaign for Trauma Informed Policy and Practice to drive national change and create resilient, trauma-informed communities where all individuals and families have the opportunity and supports necessary to thrive.
