Background Statement

Open Options was incorporated in December 1977 with a focus of providing residential services to adults with developmental disabilities. The organization began its operation with a seven-bed group home in Kansas City, Missouri in the fall of 1979, and added three more homes by 1985. A range of support was provided to the adults living in these homes. A major expansion took place in 1987 with the addition of a ten-bed home serving individuals with Prader-Willi Syndrome (PWS). This home marked the beginning of a specialized statewide program for individuals with a diagnosis of Prader-Willi Syndrome.

Open Options supported the important shift in residential services for people with disabilities as “Individualized Supported Living” became the preferred funding model in Missouri in 1990. Supported Living separates services from facilities, limits households to three persons served, and establishes customized staffing supports for people living in a home of their choice. Beginning in 1990, Open Options assisted many persons served living in group home with moving from houses with seven to eleven people into apartments with one or two roommates. These smaller settings allow for more individualized, person-centered supports and represent a positive philosophical shift away from larger group living environments.

In 1992 the mission of Open Options was expanded to include services to people whose capacity for independent living is challenged by illness or age, as well as disability. This brought the addition of the SeniorLink program that provides assessment, referral, coordination and follow-up of a cadre of senior services to help people remain independent in their homes. This program presently serves over 80 senior citizens in Clay County, Missouri.

On January 1, 1998, Open Options, Inc. and United Cerebral Palsy of Greater Kansas City completed a legal merger. In February, 2000, the organization registered with the Secretary of State to “do business as” United Cerebral Palsy of Greater Kansas City.

In January 2015, the organization rebranded as “Open Options UCP” for an interim period of time before permanently returning to the Open Options name in December 2015. At this time, the organization discontinued our affiliation with the UCP national organization, believing the Open Options name best reflects the diversity of the people we serve.

Impact Statement

We seek to become the premier provider of supports and services for people with disabilities, signifying quality, innovation, and person-centered principles. We are committed to ensuring the sustainability and growth of our organization and are driven by a ceaseless desire to improve. The people we employ will be engaged, dedicated, and invested in our mission.

 
Recent accomplishments include:
 
  1. Sixty-one people with intellectual and developmental disabilities received services from our Community Living program. Forty-eight people received full residential support in either one of our six group homes (of four residents or fewer) or in an individualized supported living setting. Thirteen people received supports through our Community Integration program, which helps people with disabilities fully engage in meaningful activities in their communities.
  2. 1,218 elementary school aged children were educated about disabilities through our Kids On the Block puppet show.
  3. Sixty-six families received targeted case management services to help them manage a disability diagnosis of a child or loved one.  
  4. Information and referrals were given to 187 people. Twenty-one people received financial assistance for assistive technology, with $16,614.02 in total awarded.
  5. Eighty-five seniors received independent living services through our Senior Link program. 

Goals for this year include: 

 
  1. Converting our Community Living records to an electronic system (THERAP);
  2. In continuing with our mission to support people to realize their full potential, our Quality Enhancement Office will work with persons served to help them be more successful in accomplishing their individual goals by using a formalized assessment process. 
  3. Individuals will be supported to attain physical wellness using tools that increase self-direction, independent decision making and direct communication with health care providers. Each person served will work with their health services coordinator to develop a preferred provider profile and a personal health plan document to help them achieve this goal. 
Needs Statement

Open Options needs the following to promote our mission to support people to realize their full potential.

  1. Our biggest financial challenge continues to be the level of government funding and rates for our contracted services; reimbursement rates simply do not cover the actual cost of providing services. As the cost of living increases without adequate rate increases, this gap grows wider.
  2. Funding to improve physical accessibility in our group homes to accommodate aging residents.  This funding will increase safety and satisfaction of our persons served.  
  3. Funding for family grants.  Hundreds of families a year seek help with the cost of assistive technology, adaptive equipment, and accessibility enhancements -  most of which are not covered by health insurance plans.  These funds are intended to purchase wheelchairs, durable medical equipment not covered by health insurance, adaptive equipment, vehicle modifications, and home modifications for those with a physical disability.
  4. Improving technology to better meet the needs of of persons served through electronic case records. 
  5. Develop a volunteer program to connect persons served with the community. Such a program would help persons served achieve personal goals while also providing a meaningful experience for volunteers. The program would also help promote disability awareness.

CEO/Executive Director/Board Chair Statement Research has shown that one in five Americans has a disability, and we are all impacted. At the same time, one of our biggest challenges is to broaden constituent support and educate the public, as we continue to raise funds for people with all types of disabilities. As Open Options continues to promote interdependence within the community, we continually tell our story, of how our services are changing the lives of all who come into contact with us. We must increase our donor base so the agency can continue providing services to individuals with disabilities through, sports and recreation programs, community living supports and homes, and information and referral connections. The generosity of the community, individuals, and foundations has given Open Options the privilege of providing over 60 years of excellence in the Greater Kansas City area. You can join us in advancing our mission in a variety of ways: making a donation, soliciting a sponsorship, participating in an event, volunteering, and by spreading the word by telling a friend or loved one about our services. 
Programs
Description

Open Options offers a variety of community living options for adults with disabilities. Community Living services assist people with sustaining a home and a lifestyle in the community.  Through individualized planning, our programs are designed to provide opportunities for consumers to participate in and & be contributing members of their communities. Some of the specific services offered in the community living program are:

 
  1. Group Living- Services are provided in homes of 4-5 people where support staff is available 24 hours per day. Open Options owns or leases these houses.
  2. Supported Living - This program allows people to establish their own homes outside of group living with one to three roommates.
  3. Community Integration Services - People with less intensive needs can benefit from periodic staff support based on a targeted plan.
 
Within these programs we have (over 20 years) established specialized services for people with Prader-Willi Syndrome. PWS is a complex genetic disorder that occurs once in every 12,000-15,000 births.

Short-Term Success Open Options UCP measures short-term success based on the people we serve's everyday ability to define how they chose to live their lives. Consistently striving to meet the needs of the consumers is a daily goal for staff. We focus on finding them jobs in the community, forming positive relationships with families, exploring the community, as well as taking trips. Community Living emphasizes living a healthy lifestyle that is developed around Person Centered Plans. These plans allow persons served to define how they live their life, and our staff works with them to ensure positive outcomes.

• Choose and direct the services and supports that will help them live meaningful lives

Examples of Program Success

We work hard to help people define what constitutes "quality of life" for the people we serve individually and increase participation in and contribution to their community.  Evidence of success often comes in small but real-life stories.  James, a 35 year old man living in a group home, loves soccer and spoke of his dream to coach soccer.  Our staff matched James with a sixth grade team who accepted him as an assistant coach for this fall season.

  • Two employee recognition programs were instituted in 2011.
  • We conduct Q12 engagement surveys will all staff after 90 days on the job.
  • Supervisory relationships are improved with use of a formalized feedback system.
  • Increased competency through the addition of 15 hours new employee training and 20 hours of supervisory training.

THE PRIMARY GOALS of the Family Support program are to strengthen individuals & families in Kansas City by providing supports to increase a family's ability to live well within the local community and also to educate the public about intellectual and developmental disabilities.

Short-Term Success Open Options understands that people’s financial situations and needs are constantly changing in terms of obtaining necessary equipment. We help these individuals obtain a financial package that will meet their needs and provide them the opportunity to enhance full citizenship in the community. Open Options also encourages people to be positive about their situation in life, making the most of what they have been given. Family support works to change attitudes about disabilities. Through community education, the organization strives to encourage people to see the ability rather than the disability.
Long-Term Success The ultimate goal for clients of family support is to be completely integrated into the community, through jobs, volunteer opportunities, and living situations. Full inclusion into mainstreamed activities and lifestyles is very crucial to clients becoming independent members of the community. Open Options also provides assistance to help clients choose their path in life, and offers support to help consumers achieve the lifestyle of their choice. It is also critical to the long-term success of the program that persons with disabilities be accepted by their families and the community, in which they live and work.

KC Power & Speed Co. has provided many opportunities to train for regional, national, and international competition. This program demonstrates the mission of Open Options, by creating opportunities for athletes to be proficient enough to excel in their chosen sport and gain national attention.  One of the successes of the organization and its clients is the ability to provide necessary resources to help people obtain what they need to remain independent productive members of the community.

Description
SeniorLink connects older adults within the Greater Kansas City community to a network of independent living services that assist them in maintaining and maximizing personal independence, increasing life satisfaction and decreasing health risks. SeniorLink, since 1993, has provided elder care consultation to seniors and their families to help them maintain the best quality of life possible. We know that the segment of our population aged 60 or older is growing rapidly. Of those over age 85, more than 40% are estimated to be in need of some type of in-home assistance. Simple tasks that one may have performed as a matter of course like housekeeping, grocery shopping, running errands or daily personal care may become more difficult or impossible. Some of the areas SeniorLink can provide assistance with are:

Short-Term Success SeniorLink works with area senior citizens that are in need of assistance to allow them to remain in their own home. The program helps them maintain independent lifestyles by connecting them to support services. Short-term success for UCP’s SeniorLink program includes creating a newfound happiness and independence that comes from an individual’s ability to remain independent, with the help of others, to stay in his or her own home.
Long-Term Success SeniorLink helps keep seniors in their homes longer and helps them to maintain independent lifestyles by connecting them to home support services. SeniorLink provides practical solutions for seniors needing assistance staying at home. Through constant monitoring and assessment, UCP remains aware, and capable of making changes that are necessary to maintain an independent lifestyle in their own homes.
Program Success Monitored By UCP regularly assesses program effectiveness and impact through feedback from clients, surveys, and home visits. SeniorLink case mangers regularly consult with vendors to ensure clients are receiving proper care, and their needs are being met.
Examples of Program Success
An elderly woman in her 80s who was in great need of a new washer and dryer recently witnessed the dedication of a UCP social worker, who helped her to obtain a new washer & dryer. The elderly woman had been saddened by her situation, & inability to purchase a new washer and dryer. With the help of SeniorLink, it was only a matter of days before she was doing laundry again. She has stated, “Without my social worker, I don’t know where I would be, I just couldn’t live in a nursing home. I know my social worker cares about me, and will do anything in her power to make sure I have everything I need to stay in my home.” Another elderly woman that has benefited from SeniorLink is a double amputee, due to diabetes. She felt she had lost her independence, because she was unable to leave her home to enjoy the simple things in life. However, SeniorLink came to her rescue & built a wheelchair ramp, so she could once again be independent & enjoy life!
 
Recent Program Outcome Measurements:
  1. SeniorLink has provided support to over 200 seniors in the past year.
  2. Over 46% of seniors served by the Ina Calkins program receive services to remain in their own homes.

Experience Mr. Bustamante has over 15 years of experience making a difference in the lives of persons with disabilities.  He brings a well-rounded and holistic viewpoint to providing services,  having worked in a variety of settings including day services, sheltered employment, and community living with two local providers and two national companies.  In March 2014, Mr. Bustamante was promoted to the President/CEO position after serving as the Director of Community Living the previous three and a half years.

CEO Comments Open Options is fortunate to have a tenured senior management staff and an active, informed board of directors.  One-third of the current board has significant life or career experience with disabilities.  The board functions efficiently with monthly meetings and committee meetings, which are well attended.  We are currently working to add new board members in fields of expertise identified by the board.  We have fully revised and expanded our leadership policies, including all areas of corporate compliance and conflict of interest.
