Background Statement

TNC Community (Harry S Truman Children's Neurological Center) was originally founded in 1949 as a place to assist children in an institutional setting. During 2001-2002, TNC faced significant challenges. With extensive Board of Directors' guidance and parent/guardian involvement, TNC has successfully met the challenges, embraced the opportunities and in so doing dramatically improved the quality of life for 38 persons receiving services.

In early 2001, TNC's Board of Directors experienced the potential loss of Medicaid Waiver funding through the Missouri Department of Mental Health (DMH) based on a continued desire by Missouri DMH not to fund services as provided in a congregate setting. Faced with the reality of ending 52 years as a service provider, TNC's Board of Directors elected to pursue the ability to offer Missouri DMH qualified residential services in individual "group homes" as community integrated facilities. Within 4 months TNC was able to locate, rehabilitate,furnish and open three additional homes. In 2002, TNC sold their long-held congregate setting at 15600 Woods Chapel Road and used part of the proceeds to finance the three newly acquired group homes (all appraised from $200,000 to $300,000).

In 2008, TNC Community in collaboration with Developmental Disability Services of Jackson County - Eitashas completed construction to the fifth group home in Independence, MO serving 6 additional individuals.

In 2010, TNC Community moved its administrative offices from the old site of the congregate setting to 12404 E.US 40 Highway, Independence, Missouri.

Presently, TNC provides supportive services to 48 adults with intellectual and/or developmental disabilities (I/DD). TNC currently operates 13 locations - five residential group homes (4-6 individuals) and eight Individualized Living Sites (ISL's, 1-3 individuals) nestled into neighborhoods throughout the Kansas City area.

The adults with I/DD that TNC assists – many who are medically fragile – need extensive, round-the-clock assistance and support as they reach a variety of personal goals. Daily living assistance and habilitation includes medication administration, personal grooming,meals, learning skills to become more independent and work, and other community activities. Through our community integration program, we strive to meet more than just basic essentials. In fact, our mission is to "advocate for interdependent living, education and community inclusion" and help provide a better quality of life.

Impact Statement

TNC is doing ordinary things daily with an extraordinary impact:
  • Maximize participation of individuals within the community, so they are engaged in a variety of activities.
  • Maintain our third 3-year CARF Accreditation.
  • Support individuals as they learn life skills to become contributing community members.
  • Remodeled a group homes to allow individuals easier access to the various areas of their homes.
  • Obtain adaptive equipment that facilitates individuals becoming more independent.
  • Encourage individuals to make progress and achieve their goals.
  • One example of this is helping an individual who is deaf to locate and attend a church that uses sign language. We also assisted some individuals with obtaining IPADs for various communication and activities.  
  • We continue to incorporate a concept known as “Gentle Teaching” throughout our organization and our homes,by training employees as "Gentle Teachers" to begin establishing a"Gentle Culture.”
  • Empower individuals in their planning process through encouragement and assistance in directing their own meetings and making the plans very personal.
  • Receive positive satisfaction survey results tallied from the individuals we support, families/parents/guardians, staff and stakeholders were incredibly positive (more than 90% of individuals surveyed noted high satisfaction.)

    1. In June, TNC hosts the Dirty Duo, a six-mile running/biking adventure race and obstacle course. Over 100 volunteers are needed for to help with food/drinks, volunteers, gather sponsors, promote the race, packet pick-up, course layout and obstacle design, prizes, racer bags & inserts, vendors, advertising, racer registration, chip timing, signage.
    2. In September, is the annual TNC Day, a family-friendly event where staff, individuals and family members get together for a fun-filled picnic with games, activities and food. Approximately twelve to fifteen volunteers are needed to help with food stations, games, setup & decorations, and tear-down.
    3. Additionally, volunteer opportunities throughout the rest of the year include help with outings or just spending some one-on-one time reading or playing games with individuals.
  4. Funds for additional staff to assist with in-home activities and community integration outings, educational training funds for consumers, families, and staff.
  5. Funds for capital improvement projects (group home renovations in kitchens, baths and accessibility projects, etc.).
  6. New or gently-used household items and furnishings for individual supported living homes.

CEO/Executive Director/Board Chair Statement TNC Community has a proud history of serving people with intellectual and developmental disabilities for over 65 years. TNC was born out of the United Cerebral Palsy Association in the early 1950's. Until 2001 TNC operated a large conjugate care facility in eastern Jackson County as well as group home in Raytown and another in Kansas City North.  In the fall of 2001 TNC worked with the Department of Mental Health to close the large facility and move into a group homes model.  Three wonderful new facilities were purchased and converted to group homes, each one housing six residents.  The North Kansas City home was sold and replaced by a much newer and more functional facility.  TNC's board and staff worked closely with DMH, the Jackson County Board of Services and the Clay County Board of services to develop a system of residential care that is entirely consumer focused and centered.  A primary goal of the organization is the hiring and retaining of high quality direct care staff and solid middle management.  Our full time training director and program management does an excellent job of training staff to make them ready for the challenges they face.  This personalized training regimen allows new staff to feel better prepared to go to work serving our consumers.  The result is better care for the consumers and a very low turn over rate due to high job satisfaction.  At the same time we have continued to upgrade our facilities in each of our neighborhoods and we have accomplished that goal.  Our cash reserves put us in the best of class of all comparable organizations.  We have continued to upgrade and reconstitute our board into a diverse and effective governing body.  The board consists of three supported individuals, a parent and an excellent collection of other professionals to give guidance to the organization.  TNC works closely with our Executive Director to plan the organization's future.

Program Success Monitored By TNC utilizes an Outcome Management reporting system to monitor the programs success. Satisfaction surveys are completed annually by consumers and/or guardians.
Examples of Program Success

JM, who is non-verbal, is was an individual whose social interactions were modest at best and his communication was at a low minimal.  He focused mainly on sitting in a recliner and rocking.  As the staff of TNC Community continued utilizing Gentle Teaching techniques introduced at a workshop attended by a program manager staff member.  Since the workshop, JH has been receiving continous one on one attention.  As a result, JH social interactions have increased tremendously.  For example, JH visits the administrative office with staff on occasion and he now likes to visit with every person in the office.  He will enter the office and sit in any chair that is available waiting for you to get up and offer him your hand to walk around the office.  JH will now attend community events feeling safe and secure and without verbal aggression.  CE is another consumer that resides at TNC Community who has always wanted a job.  CE with staff assistance is now employed with the humane society and works Saturdays and Mondays at the local shelter.  CE is so extremely happy.  He is always ready for work.

CEO Comments

A primary challenge that our organization faces is aging and declining health. As the individuals that we support continue to age, their health becomes increasingly more complicated. As their needs increase, TNC’s cornerstone is its quality of care. Despite all of our individuals' health complications, TNC’s staff continues to work with each individual to help them meet their goals. From helping one individual gain the right to vote to helping others learn to walk to helping individuals learn critical social skills that will help them navigate within the community. These goals, while small, are vitally important to each individual’s self esteem. And as they continue to make progress on their goals,they’re working towards self determination, a concept that encourages individuals to become the driving force behind their life.

This past year, 72% of individuals assisted made progress on at least 85% of their goals. TNC presently employs three Community RNs and three Health Support Coordinators to help with our aging population.

As these needs continue to increase, our agency is committed to fully supporting the individuals we assist. But it’s important to remember that the state only reimburses the minimum care. It’s clear that the needs of our individuals surpass the minimum level of care.

Experience The Executive Director of TNC Community is James Landrum. Mr. Landrum was retained as Executive Director after serving 5 months as Interim Executive Director for TNC Community. He has faithfully served TNC Community in various roles (Qualified Mental Retardation Professional, Program Director, Director of Operations) since joining the agency on 3/5/2001. He knew at a very young age that his love and dedication would be working in the human service area. He was previously employed in retail management where he developed skills in working with the public. He was with Aiken Regional Medical Center as a Mental Health Technician for three years. He also served as a Coordinator with the Marketing Department at Aurora for two years. He had worked in the mentally and physically challenged field from 1993 to 1997 as a Work Crew Supervisor and Job Coach. In 1997 he worked with Richmond/Burke Job Training Partnership as a Welfare to Work Case Manager/Coordinator. His work with the economically disadvantaged was according to James, "challenging yet rewarding." Mr. Landrum is a native of South Carolina. He relocated to Kansas City in February of 2000 where he continued to work with Welfare Reform. James has a direct, yet sensitive and caring approach to clients and staff. He offers client tools in their quests to become self-sufficient. He is resourceful and intuitive and enjoys problem solving and helping others. James has completed his Masters in Social Work at the University of Kansas. James was named in Ingram's 2014 Class of 40 under Forty.

Ardith Scheer, RN, CDDN, is the Director of Health Services. Ardith has an extensive background in the medical field – from hospital nursing to corporate nursing to public and home health. She is a past recipient of Eitas’ (Jackson County Board of Developmental Disability Services) Employee of the Year and a Gentle Teaching Mentor.

Ginni holds a Bachelor of Science degree from Lamar University in Speech Pathology/Audiology. A native of Texas, Ginni relocated to Missouri in 1985. In addition to superb work on payroll and benefit administration, Ginni also manages the personal funds of all TNC clients. Ginni maintains all of TNC client archive file management.

Organization Comments TNC provides accurate and timely monthly financial statements. All accounting practices are conducted within a code of ethics and may be analyzed as applying to generally accepted accounting practices. TNC now has a 3-month operating reserves. TNC operates under an annual budget that has been meticulously prepared and analyzed by the Executive Director and Finance Director. Budget versus actual comparative analysis is performed monthly and reviewed by the TNC Board of Directors and remedial action is implemented, if needed. TNC financial statements are audited by a CPA firm, reviewed by Commission on Accreditation of Rehabilitation Facilities (CARF), and successfully submitted to local financial institutions. TNC’s management has implemented policies and procedures to effectively manage cash flow and accounts payable. TNC also continues to seek funds through private grants and donations to assist with the operation and implementation of programs. Thanks to the consumers, parents/guardians, staff, friends, and Community for their dedication and support!
